Print out a copy of our booking form, complete your personal details and then ask your LMC to complete the medical information. Once completed, the form can be scanned and emailed to [email protected] or posted to 56 View Rd, Warkworth 0910. Please make sure all areas are filled in and the form is signed by yourself and your LMC. Note: If you were not born in NZ, we do require a copy of your passport and residency visa/work permit information to accompany your booking form.
